Hello, I am Tony.
I got permanently banned since months.
But I can see and I know that I can get unbanned again, because of this rule:

Inappropriate Skins / Capes​

The use of Inappropriate Skins or Capes is strictly disallowed on the Network. Additionally, players using inappropriate, offensive, or/and sexual skins/capes will be banned from the Network.
Note: For somebody to be punished under this rule, the account using the skin/cape must be premium.
First Offense ➟ Permanent Ban

and i am cracked + The person who named TonyIstOP changed his / her name

If i want to write an appel again it says: You have already reached the limit of the number of times you can fill out this form.

My Question is how I can fill the form out again

You can also contact other staff and they can let management team members know for you

About you skin problem, I had a discussion about it once with Rodagave and it is understandable that as this is a cracked server anyone can buy your IGN if available and change your skin to something inappropriate.
In your appeal make sure to mention this and request for a server side skin change
