You have to have approved 15 gameplay reports and 20 approved chat report. To get to know jaryex rules.

I hope I can clear up some of your doubts.

1) Yes, you need to have at least 15 accepted gameplay reports, and at least 20 accepted chat reports.

2) The age limit for staff is mentioned in the requirements sub-section of the application form, in a roundabout way. The form states "You must meet the age required by your country to use Discord."
Given that the minimum age for Discord in any country that does not have other restrictions is 13, consider this the minimum age. If you are in a country such as Ireland for example, the minimum age is 16, therefore you would need to be 16 in order to meet the minimum age for staff.
